Decreases in arterial blood stress enhance sympathetic tone, enhance adrenal secretion of epinephrine, and suppress vagal activity. The resulting systemic vasoconstriction, elevation in heart fee, and enhanced cardiac contractility enhance blood pressure. Peripheral baroreceptors are situated on the bifurcation of the widespread carotid arteries and the aortic arch. Elevations in blood pressure increase baroreceptor discharge, inhibiting systemic vasoconstriction and enhancing vagal tone (baroreceptor reflex). After perfusing the myocardium, blood returns to the right atrium through the coronary sinus and anterior cardiac veins. Increases in coronary heart fee also lower coronary perfusion due to disproportionately greater reduction in diastolic time as coronary heart price increases. The endocardium is most vulnerable to ischemia throughout decreases in coronary perfusion pressure. Whereas vasodilation attributable to desflurane is primarily autonomically mediated, sevoflurane appears to lack coronary vasodilating properties. These pressures are transmitted to the left atrium and pulmonary vasculature, resulting in signs of congestion.
